OSCAR eForm Creator (“the extension”, “we”) is a browser tool for building OSCAR EMR electronic forms (eForms). This policy explains what data the extension does and does not handle.

1. Forms and images you work with — processed locally, never collected

The form images you upload and the fields you place are processed entirely within your own browser. They are never uploaded to us or any server, never transmitted off your device, and never stored by us. We do not see, access, or collect your forms or any patient, health, or clinical information.

2. Subscription data (via ExtPay)

Access to the extension is managed through ExtPay, a third-party service that uses Stripe to process payments. When you start a subscription or free trial:

• Your email address is collected by ExtPay to create and identify your subscription.
• Your payment information is collected and processed by Stripe through ExtPay’s secure checkout. We never receive or store your full payment card details.

The extension stores your subscription status locally in your browser (via Chrome storage) so it knows whether to unlock the tool.

3. How we use data

We use your email address and subscription status solely to provide and manage your access to the extension. We do not use them for any other purpose.

4. What we do not do

• We do not sell or transfer your data to third parties, other than the payment providers above and only to process your subscription.
• We do not use or transfer your data for any purpose unrelated to the extension’s single purpose.
• We do not use your data to determine creditworthiness or for lending.
• We do not collect analytics, browsing history, location, personal communications, or health information.
